Volunteer Informed Consent Agreement
Release of Liability, Assumption of Risk & Agreement to Pay Claims

In working with PEP-Hawaii, an organization that operates under the auspices of the Prison Education Project [PEP] and the 501(c)3 Transcendence, I understand and agree to the following statements.

I voluntarily assume any and all risks and agree to pay any and all claims arising from or incidental to my participation in PEP-Hawaii volunteer service activities/events including those that may result in:
a. Personal injuries including medical or hospital bills, permanent or partial disability or death.
b. Damage to personal property caused by or arising from participation in the volunteer service.

I will submit to any required background checks and respect the rules of all facilities.

I grant the Prison Education Project the right and permission to use photographs, film, tape or sound recordings taken of during the course of volunteer service for use in non-print and print media presentations.

I may be removed from participation in PEP-Hawaii at the discretion of PEP leadership.

The Prison Education Project has no control over information used by third parties.

I will model professional and appropriate behavior when working in association with PEP-Hawaii.

I understand that the PEP-Hawaii experience is coordinated by the Prison Education Project and is in no way affiliated with a university.
As a condition of my volunteer service, I release the Prison Education Project, and Transcendence Children and Family Services and their employees, officers and agents from Liability for personal injury, wrongful death and property damage arising from participation in any events or activities associated with PEP-Hawaii. This document reflects my understanding of the relationships. My electronic signature and submission of registration forms for volunteering with the Prison Education Project in Hawaii confirms my understanding and consent for release of liability.
